UCSF Postgraduate Certificate in Pain Management / Online Program

To Register

  1. Submit your completed UCSF Graduate Application form and CV online plus required supporting materials by mail (see # 2, 3, and 4 below), and also pay the appropriate UCSF Graduate Application Fee ($60 U.S.; $80 International; non-refundable). (Click on "New Application", complete the form, and select "Certificate in Pain Management" under the section "Program You Are Applying to".)

    Deadline to ApplyThe completed application form, application fee, and supporting materials must be received by UCSF on or before August 1, 2008 (received by this date, not postmarked).

    ** NoteThis online application form does not need to be completed in one sitting.  You will be permitted to return to the application form online as many times as you need – until you have clicked on the button to “Lock the Application”.  Just remember to SAVE the portion of the application you’ve completed before logging out; otherwise, your work will be lost.  Read the instructions on each page of the application form thoroughly before completing each section of the application.  (No changes will be permitted after you have locked the application so be sure to proofread your application at least once after you’ve completed and saved everything, making any last changes you need – before you lock the form.)  Also, be sure to have a brief copy of your CV ready in PDF format for attachment to the online application form.  (See online application form for instructions.)  Then, follow the instructions for locking your application and paying the required UCSF Graduate Application Fee as indicated, and proceed to steps 2-5 below.
  2. Have the university awarding your highest degree relevant to this program send an official transcript (unopened and displaying the university seal) to the program administrator, Laurie Snyder, at the address shown at the top of this web page.

    *** NoteIt is critical to request transcripts early so that they arrive before the application deadline of August 1, 2008.
  3. Then, have your TOEFL (Test of English as a Foreign Language) score report sent to UCSF by Educational Testing Service (if applicable)*.  Call Educational Testing Service (ETS) to request that they send your official TOEFL score report directly to UCSF so that it will be received by UCSF before August 1, 2008.  Use the Institution Code R4840.  See the ETS web site for fees and instructions regarding how to request your scores by phone or e-mail:

  4. Then, forward the required Recommendations Form to your professional references (2 minimum), and ask that they return their evaluations of you to UCSF before August 1, 2008.  (See the online application for a downloadable PDF of this form; see the form itself for the return UCSF mailing address.)
  5. Upon official notification from the program administrator that you have been accepted into the program, arrange for payment of your tuition with the UCSF Registrar.  Tuition may be paid in full or in 3 quarterly installments by credit card, check, or money order, and must be paid on or before the deadlines designated by the UCSF Registrar.  (Information regarding the specific procedures for payment of your tuition and instructions for how to begin your online studies will be provided by the program administrator when you are notified of your acceptance into the program.)
