Consultation Services

Overview of Services:

We accept wet tissue specimens, glass slides for consultation, immunofluorescence specimens, and (by prior arrangement) frozen tissue specimens. All specimens should be forwarded with a completed requisition form or a consultation letter spcifying the following:
  • name of the patient
  • date of birth
  • social security number
  • site of biopsy
  • date of biopsy
  • clinical context

We supply all specimen containers and requisitions. Supplies can be requested directly from our office by telephone. Please use our distinctive immunofluorescence vials, which have bright green caps to permit distinction from formalin-fixed specimens. All specimen containers should be labeled for identification.


Insurance Information:

Please specify who is to be billed. If the patient or the patient's insurance is to be billed directly, please include the patient's full name and home address and telephone number, as well as all identifying data discussed above. To bill insurance accurately, we need one of the following:

  1. Name and address of insurance company with group or policy number;
  2. HMO name with authorization number;
  3. Medicare number with name and address of secondary insurer; or
  4. Medi-Cal number and current month sticker.

If the patient is insured by a reduced fee-for-service or capitated contract with UCSF or our service, identification of the specific contract is sufficient. A photocopy of an insurance card is helpful.


Specimen Transportation:

We are happy to assume all transportation costs, with the exception of items requiring exceptional handling, such as frozen tissue specimens.

  • Within the San Francisco area, we provide daily courier service, and specimen pickups can be scheduled by calling our office. Most specimens can be picked up the same day, and all specimens will be picked up within 24 hours.
  • Throughout California and the United States, wet tissue and/or slides can be forwarded to us via Federal Express. The phone number for FedEx is 1-800-238-5355.
  • International consultations must be forwarded at the expense of the sender.
